Restaurant kids' menus

I've been perusing the menus in advance of our November trip (love being able to check out food and prices), and the kids menus either say 9 and under, or they don't have an age specified at all.

How strict are they? I have an 11 year old who is tiny and can easily pass for 9 at a restaurant, but I also have 13 year old who may on occasion prefer something from the kids menu. Would this be allowed?

I haven't done much travel with kids, but I've seen tables full of just teens all order kids meals and no one cares
